Teacher Leadership Academy Application

    Thank you for your interest in the Teacher Leadership Academy! Before starting the application, please review the following program session dates and confirm your availability, as participants are expected to attend each session: • August 19–20, 2026 • October 14, 2026 • November 18, 2026 • December 9, 2026 • February 17, 2027 • March 17, 2027 • April 21, 2027 • June 9, 2027 If you have questions about the program or eligibility, please contact Tyronna Hooker, VP of Education Programs, at tyronna.hooker@conehealth.com . After submitting your application, you will be directed to a confirmation page and will also receive a confirmation email at the address provided in the form. All applicants will be notified of their status on June 5. Selected participants will have until June 11 to confirm their participation. If you choose to save your application and return later, please follow the instructions on the linked page to receive your unique access link. You will be prompted to complete any required fields before submitting your application.

    Please write a brief response to each of the questions below. The selection committee uses your answers to get to know you and better understand what motivates and inspires you. Take some time to reflect on the values, interests, and experiences that have shaped you.
  • Please describe an effective teacher leader and how you have demonstrated these traits with your students and peers. Responses should be between 240 and 400 words.
